Use the Ishikawa Diagram to Map the Root Causes of Problems and Nonconformities
The Ishikawa Diagram, also known as the Cause-and-Effect Diagram or Fishbone Diagram, is a visual root cause analysis tool used to systematically investigate recurring problems. To use it, start with an effect or failure and map it graphically, identifying all possible causes and subcauses organized into specific categories—typically Manpower, Machine, Method, Material, Environment, and Measurement. This systematic approach helps prevent superficial analyses and contributes to more effective corrective actions by providing a holistic view of the factors influencing the problem.
